Job Description
As a Licensed Practical Nurse (LPN) with IntelyCare, you will be responsible for delivering post-acute care to residents. This includes monitoring their physical and mental wellbeing, tracking vital signs, performing intakes, and documenting health concerns and medical histories.
You will also be responsible for reviewing medical records and charts, helping to maintain appropriate levels of medical supplies, and contributing to a safe and comfortable environment for residents. Additionally, you may be required to assist with IV set up, vaccinations, and phlebotomies, as well as follow and adhere to care plans for residents.
