Home Health Nurse

We are seeking a skilled Registered Nurse to join our Home Health team in Northampton, Massachusetts. This is an excellent opportunity to provide high-quality care to patients in the comfort of their own homes.
Job Responsibilities- Provide skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
- Assess patient needs and develop individualized care plans to promote optimal health outcomes.
- Collaborate with interdisciplinary teams to coordinate care and services.
- Graduate of an approved, accredited school of professional nursing required; BSN preferred.
- Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
- Minimum of one (1) year Medical/Surgical nursing experience required; home care/hospice experience preferred.
- Demonstrated excellent verbal and written communication skills and strong interpersonal skills required.
The estimated annual salary for this position is $85,000-$110,000, based on experience and location.
