The Nurse Care Manager will be responsible for the management and delivery of patient care to assigned patients in collaboration with the interdisciplinary team, and within the scope of practice delineated in the New York State Nurse Practice Act. The ideal candidate will provide direct patient care, delegate/assign responsibility as appropriate to others, and serve as leader of the interdisciplinary team in collaboration with and under the direction of the attending physician. Key responsibilities include completion of the nursing process including patient/family education and discharge planning, as well as understanding and acting appropriately to ensure optimum use of all resources.
Requirements:
- Bachelor's degree in Nursing is preferred; new hires will be required to complete their BSN within 5 years of hire.
- Graduation from a Registered Professional Nursing Program.
- One year of medical-surgical or acute care/hospital nursing experience required.
