Job Title: Home Care Manager RN

Job Summary:

The Home Care Manager RN is a key member of the Hudson Headwaters Health Network team, responsible for providing high-quality home care services to our patients. This role requires strong clinical and leadership skills, with a focus on delivering patient-centered care in a home setting.

Key Responsibilities:

  • Assess and develop individualized plans of care for patients in collaboration with the interdisciplinary team.
  • Conduct regular assessments and evaluations to ensure patient needs are met and care plans are updated accordingly.
  • Provide education and support to patients and their families to promote self-care and independence.
  • Collaborate with healthcare providers and community resources to ensure seamless care transitions and coordination.
  • Lead and manage a team of home care professionals to deliver high-quality patient care.
  • Develop and implement quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Ensure compliance with regulatory requirements and organizational policies.

Requirements:

  • Associate's degree in nursing required; Bachelor's degree in nursing preferred.
  • Current New York State RN license.
  • 2+ years of geriatric nursing experience.
  • BLS certification or willingness to obtain.
  • Certification in PRI and UAS-NY assessments.
  • 4+ years of supervisory experience in healthcare, preferably in a home care setting.
  • Valid driver's license.
  • Excellent communication, leadership, and problem-solving skills.
  • Ability to work independently and as part of a team.

Physical Requirements:

This role requires the ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA, and other federal, state, and local standards. The employee must be able to exert 50 pounds of force occasionally, up to 20 pounds of force frequently, and up to 10 pounds of force constantly to move objects. The employee will require 20% sitting, 30% walking, and 50% standing. May also need to move heavy equipment, transfer/lift patients, and assist patients with walking and turning.

Compensation:

The pay range for this position is $78,500 - $87,500 annually, based on skills and experience.
