We are seeking a highly skilled Care Coordinator to join our team in Ellis, Russell, and Barton Counties in Kansas. As a Care Coordinator, you will play a critical role in ensuring that individuals receive the necessary services to achieve their goals and live independently.

Key Responsibilities
  • Assess and develop individualized care plans that address the unique needs and goals of each person.
  • Coordinate with healthcare providers, community resources, and family members to ensure seamless care transitions.
  • Identify and initiate referrals for social services, including financial, psychosocial, and community-based programs.
  • Ensure that care plans include covered services, EPSDT, and value-added benefits.
  • Communicate effectively with stakeholders to ensure quality coordinated care and services.
Requirements
  • Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related field.
  • At least one year of experience working with individuals with long-term care needs.
  • Intermediate level of computer proficiency, including Microsoft Outlook and Teams.
  • Ability to travel 75% of the time and have a valid driver's license.
Preferred Qualifications
  • Licensed Social Worker or clinical degree.
  • Experience working with individuals receiving services on home and community-based waivers in Kansas.
  • Background in managing populations with complex medical or behavioral needs.
