Responsibilities
  • Provide clinical case management functions to patients, including assessment, planning, and implementation of care plans.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Develop and maintain relationships with patients, families, and healthcare providers to ensure comprehensive care.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Master's Degree in Social Work (MSW) from an accredited institution.
  • Washington Clinical Independent Social Worker Associate License (upon hire).
  • 1 year of directly related work experience in social services in a healthcare setting (preferred).
  • Experience in crisis intervention (preferred).
