Job Summary:

As a Home Health Clinician with VNA of Care New England, you will play a vital role in managing a caseload of patients from admission to transition into the community. Working under the supervision of the Team/Program Manager, you will be accountable for coordinating interdisciplinary care, monitoring and adjusting treatment to achieve positive therapeutic outcomes, and identifying opportunities for process improvement.

This position requires a strong clinical background, with a minimum of one year of progressive clinical experience. Previous experience in an acute care or Home health care environment setting is preferred. You will be responsible for reading and interpreting medical records, reports, and other documents, as well as writing reports, business correspondence, and procedure manuals.

Responsibilities:
  • Coordinate interdisciplinary care for patients

Requirements:
  • Equivalent education required for RI Registered Nurse licensure
  • Valid RI Registered Nurse license
  • A minimum of one year progressive clinical experience
  • Previous experience in an acute care or Home health care environment setting preferred
  • Valid CPR certification
  • Valid driver's license and automobile liability insurance
