We're seeking a skilled Registered Nurse (RN) to join our team as a Case Manager in Home Health. As a national leader in home-based care, Enhabit is committed to expanding what's possible for patient care in the home.
As a RN Case Manager, you'll be the point of contact for all disciplines involved with providing care to patients and oversee the frequency of visits for the episode. You'll consult with physicians and office staff to ensure accurate patient care, perform OASIS collection, and maintain current medication profiles.
Key responsibilities include:
- Performing visits and coordinating client care
- Overseeing visit frequency for episodes
- Consulting with physicians and office staff
- Accurate OASIS collection and medication profile management
Qualifications
To be successful in this role, you'll need:
- Current licensure as a Registered Nurse (RN) in the state of employment
- A minimum of one year of clinical experience
- Demonstrated knowledge and skill in current nursing practice
- A valid state driver's license and automobile liability insurance
