Overview

At Enhabit Home Health & Hospice, we're committed to delivering exceptional patient care in the comfort of their own homes. As a Registered Nurse (RN) Case Manager, you'll play a vital role in coordinating patient care and ensuring the highest level of quality and compassion.

Responsibilities

As an RN Case Manager, your key responsibilities will include:
  • Performing visits and coordinating client care to ensure seamless delivery of services
  • Overseeing the frequency of visits for each episode and consulting with physicians as needed
  • Accurately collecting OASIS data and maintaining up-to-date medication profiles
  • Attending weekly case conferences and monthly case manager meetings to stay informed and aligned with patient care goals

Qualifications

To be successful in this role, you'll need to meet the following requirements:
  1. Be currently licensed as a Registered Nurse (RN) in the state of employment
  2. A minimum of one year of clinical experience is preferred
  3. Demonstrate knowledge and skill in current nursing practice
  4. Possess a valid state driver's license and automobile liability insurance
