About the Role:

The University of Missouri is seeking a dedicated Nurse Navigator to become an integral part of our oncology team. This role is essential in fostering patient-centered care, ensuring seamless coordination throughout the treatment journey. The ideal candidate will demonstrate a strong commitment to patient advocacy, working collaboratively within a multidisciplinary team to enhance healthcare delivery.

Key Responsibilities:

  • Facilitate care coordination for patients with specific health conditions.
  • Serve as a resource for patients, families, and healthcare professionals, providing expert guidance in navigating treatment options.
  • Educate patients and caregivers on disease management and personalized treatment plans.
  • Lead initiatives aimed at improving quality indicators and participate in performance enhancement activities.
  • Establish processes to ensure timely access to care for patients.
  • Deliver transitional care to maintain compliance with treatment protocols.
  • Manage patient triage and symptom management, directing them to appropriate care levels.
  • Review care pathways to ensure adherence to established protocols.
  • Analyze patient outcomes to identify opportunities for improvement.
  • Provide systematic care management for at-risk populations to minimize care fragmentation.

Qualifications:

Applicants must meet one of the following criteria:

  • Bachelor of Science in Nursing with three years of RN experience; or
  • Five years of RN experience; or
  • One year of RN experience combined with five years of LPN care coordination experience.

Preferred Qualifications:

Additional licensure or certification may be required as determined by the hiring department.

Physical Requirements:

The physical demands of this position may include the ability to ambulate, maintain a stationary position, and reach or move objects as necessary. Employees may be required to lift objects up to 10 lbs.
