Job Summary

As an Oncology Nurse Navigator, you will play a vital role in coordinating the care of oncology patients, their families, and caregivers across the continuum of care. Your objective will be to provide high-quality patient care across all care delivery sites, including coordination, facilitation, monitoring, and evaluation of interventions to achieve desired outcomes.

Key Responsibilities
  • Evaluate new patient cancer referrals to initiate contact with patients and review patient records for NCCN guidelines for diagnosis, staging, and treatment.
  • Assess the health status of patients targeted for participation in cancer care management programs, conduct initial assessments, interviews, and screenings/determinizations for risk.
  • Ensure patient referrals to appropriate support services after a comprehensive assessment of patient needs and barriers.
  • Participate in the development of assessment tools, care management guidelines, pathways, and algorithms for cross-continuum care of patients with cancer at all points in the care continuum.
  • Participate in and create multi-disciplinary care plans as appropriate for patient care needs and direction.
  • Conduct patient education, collaborate with patients to establish goals and activities to enhance patient self-management and participation in the plan of care.
  • Promote and provide patient, family, staff, and community education through the use of expert clinical knowledge bases.
Requirements
  • Bachelor's Degree - Nursing
  • Minimum 3 years of Oncology experience
  • Licensed to practice as a Registered Nurse in South Carolina
  • One of the following oncology certifications: OCN, ONN-CG, BMTCN, or CBCN
