Infectious Disease Specialist

Job Summary
This role provides comprehensive care services to individuals living with HIV and other chronic infectious diseases. The physician will develop diagnostic and treatment care plans, counsel patients on their illnesses, and educate them on healthy choices.
Key Responsibilities
- Provide outpatient care consultation and ongoing treatment of chronic infectious diseases.
- Deliver comprehensive care for patient panels, including management of chronic diseases directly or through collaboration with primary care physicians and specialists.
- Work in a team environment to process feedback and facilitate open discussion.
- Implement plans of care in collaboration with patients, their families, and other team members.
- Develop strong transitional care plans and track patients along the continuum of care.
Requirements
- Unrestricted Pennsylvania Medical License
- Board certified Internal Medicine or Family Practice
- Certified by AAHIVM or Board Certification in Infectious Diseases
- BLS and DEA
- Experience in population health management and working with psychosocially and medically complex patients
