RN Care Manager

As a key member of the Hamilton Health Care System team, the RN Care Manager plays a vital role in delivering high-quality, patient-centered care to individuals with terminal illnesses.
This position requires a skilled nurse to develop, implement, and evaluate comprehensive care plans that address the unique needs of each patient.
The RN Care Manager will work collaboratively with the Hospice Clinical Nurse Manager, physicians, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
Responsibilities include:
- Providing direct patient care and education to patients and their families
- Developing and implementing individualized care plans that meet the patient's physical, emotional, and spiritual needs
- Collaborating with the interdisciplinary team to ensure comprehensive care and coordination
- Supervising and directing Hospice Home Health Aides and LPNs as needed
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced, dynamic environment.
