Key Duties

1. Assess eligibility by:
  • Conducting interviews with individuals to gather essential information.
  • Explaining available benefits and resources.
  • Reviewing application documents for completeness.
  • Assisting applicants in obtaining necessary verifications.
  • Collaborating with various agencies and organizations.
  • Processing relevant forms and documents.
  • Applying program rules to determine eligibility.
  • Providing timely notifications of eligibility decisions.
  • Maintaining comprehensive consumer files.
  • Handling incoming calls related to eligibility inquiries.
2. Conduct annual reviews by:
  • Ensuring consumers receive re-determination information.
  • Reviewing returned materials for completeness.
  • Contacting consumers for additional information as needed.
  • Applying program rules to verify continued eligibility.
  • Maintaining accurate consumer files.
  • Processing information between determinations to ensure benefits remain current.
3. Provide ongoing maintenance by:
  • Reviewing and processing updated information from consumers.
  • Adjusting benefits as necessary.
  • Verifying consumer information.
4. Offer additional assistance by:
  • Processing and reviewing forms related to the MMIS database.
  • Providing resources and community referrals.
  • Attending consumer hearings and providing testimony as required.
5. Advocate for person-centered services by:
  • Embracing the agency's mission and core values.
  • Understanding the importance of consumer choice.
  • Interacting respectfully with the public and colleagues.
  • Educating oneself and others about available resources.
6. Safeguard consumer interests by:
  • Adhering to policies and regulations.
  • Documenting plans and progress as required.
  • Serving as a mandatory reporter of suspected abuse.

Qualifications
Minimum Requirements:

A qualified candidate will possess a minimum of four (4) years of a combination of education and experience demonstrating the necessary knowledge, skills, and abilities for this role. Acceptable qualifications include:
  • A Bachelor's degree in social sciences or a related field.
  • Experience working directly with consumers in social service settings.
  • A combination of relevant education and work experience totaling at least four years.
