Asociacion Puertorriquenos En Marcha Inc is seeking a highly motivated and experienced Community Connector to join our team. The successful candidate will be responsible for performing community outreach and engagement in North Philadelphia, working with victims of non-fatal shootings and individuals at risk of perpetuating a cycle of violence.
Key Responsibilities- Collaborate with the APM Violence Intervention team to provide community engagement and outreach in the 24th and 26th Philadelphia Police Districts.
- Outreach to non-fatal shooting victims within 48 hours of an incident occurring.
- Work with Hospital-Based Violence Programs to engage with and deem eligibility for non-fatal shooting victims in their care.
- Meet varying criteria for different services offered to each young person deemed eligible within a specific time-frame.
- Carry a small caseload of non-fatal shooting victims.
- Represent APM in community events, meetings, and learning communities in coordination with other APM Program Staff.
- Collaborate with APM Directors to engage with key partners to increase referrals, resources, and supports for participants.
- Maintain the Non-Fatal Shooting Victim Outreach list.
- Collaborate with APM Directors to design plans to engage young people on a weekly basis.
- BA preferred/Equivalent experience accepted - Minimum of three years of direct service work with high-risk young people.
- Understanding and experience working with street-involved high-risk young people.
- Valid Driver's License or access to public transportation.
- Health Insurance through Independence Administrators or $100/month reimbursement with proof of current insurance.
- Vision and Dental Plans through SunLife.
- Basic Life Insurance (100% Employer Funded).
