Key Responsibilities:

  • Lead the coordination and execution of the Resident Assessment Instrument (MDS, CAA's, and Care Plan) in compliance with applicable Federal and State regulations.
  • Supervise and document the management of Medicare and Managed Care residents in collaboration with the facility's healthcare team.
  • Implement innovative healthcare solutions that enhance systems and processes for optimal outcomes.

Qualifications:

  • Must hold a current, unencumbered state license to practice as an RN or LPN.
  • Preferred experience in Skilled Nursing or Rehabilitation settings.
  • Minimum of six months of experience as a MDS Coordinator.
  • RAC-CT or RNAC certification preferred; consideration will be given to LNAC candidates.
  • Must demonstrate compassion, dedication, and a commitment to excellence in care.
