Key Responsibilities:
- Supervise and actively participate in the completion of the Resident Assessment Instrument (MDS, CAA's, and Care Plan) in compliance with current Federal and State Regulations.
- Monitor and document the management of Medicare and Managed Care residents in collaboration with facility team members.
- Deliver innovative and responsible healthcare by developing and implementing new ideas and processes to enhance systems and achieve superior outcomes.
Minimum Qualifications:
- Must hold a current, unencumbered, active state license to practice as an RN or LPN.
- Experience in Skilled Nursing/Rehabilitation facilities is preferred.
- A minimum of six (6) months of experience as a MDS Coordinator is required.
- RAC-CT or RNAC certification is preferred, but LNAC will be considered.
