Job Summary

As a Social Worker MSW II-Home Care, you will be responsible for delivering social work services to identified home care clients. This includes providing psychosocial support and counseling, facilitating financial and long-term care planning, and communicating with the healthcare team to document all care activities.

Key Responsibilities

Determine patient and family needs related to social supports, financial support, or counseling
Coordinate multidisciplinary and agency case conferences
Work with care managers to advocate for patient/family to obtain approval for insurance coverage
Coordinate admission for all patients in need of dialysis

Requirements

Master's degree in Social Work with a minimum of two years' experience in Social Work or a related field
Proven psychosocial assessment skills
Effective verbal and written communication skills
Ability to stay organized and flexible
Possession of a valid NYS driver's license and own car required
