Position Overview:
The Nurse Assessment Coordinator (NAC) plays a crucial role in ensuring compliance with regulations pertaining to the MDS and related documentation. This position is responsible for coordinating the interdisciplinary Plan of Care, optimizing service utilization based on care levels, and assisting the Lead NAC in maintaining the highest standards of quality care for all residents.
Key Responsibilities:
- Regularly reviews Medicare documentation to ensure compliance with regulations.
- Coordinates the Plan of Care with the clinical team.
- Addresses pain assessments and CAAs care plans.
- Conducts Utilization Review for Medicare NOMNC/ABN Letters, certifications, hospital referrals, and evaluates hospital records for ICD10 coding.
- Sets ARD dates and manages OBRA scheduling and Part B Authorization as necessary.
- Collaborates with other departments for accurate MDS coding.
- Engages in ongoing education for initial NAC certification and recertification every two years to stay updated on MDS coding changes.
- Follows guidance from the Regional NAC and supports team recommendations.
- Provides monthly education to caregivers regarding MDS coding.
- Assists in implementing the orientation program for new staff as appropriate.
- Fosters positive relationships with interdepartmental personnel to ensure nursing services meet resident needs.
- Creates a warm, engaging atmosphere while maintaining confidentiality of resident care information.
- Participates in weekly CMI meetings.
- Performs additional related duties as required.
Qualifications:
- Current RN license in the Commonwealth of Pennsylvania.
- Experience with MDS is essential.
