RNAC Coordinator

St. Martha Center is seeking a highly skilled Registered Nurse Assessment Coordinator (RNAC) to join our team. As a RNAC, you will play a critical role in ensuring the highest level of quality care for our residents.

• Coordinate the fulfillment of regulations relative to the Minimum Data Set (MDS) and associated forms.
• Develop and implement the interdisciplinary Plan of Care to ensure appropriate utilization of services relative to levels of care.
• Assist the Lead RNAC in ensuring the highest degree of quality care is maintained at all times for each resident.
• Review current Medicare charts on a regular basis to ensure documentation compliance of Medicare Regulation.
• Coordinate information with other departments when coding MDS.
• Provide ongoing education for initial RNAC certification and recertification requirements every 2 years.
• Develop and maintain a good working rapport with interdepartmental personnel and other departments within the facility.
• Participate in weekly Clinical Management Information (CMI) meetings.

• Current RN license in the Commonwealth of Pennsylvania.
• RNAC Certification.
• Experience in MDS coding.
