RNAC Coordinator

Welcome to St. Martha Center for Rehabilitation and Healthcare, a 120-bed community providing a warm and caring environment for short-term rehab patients and long-term residents, including memory care.

Our team of seasoned staff work together to offer excellent service and care to each resident. The Registered Nurse Assessment Coordinator (RNAC) plays a crucial role in ensuring the highest degree of quality care is maintained at all times.

• Reviews current Medicare charts to ensure documentation compliance with Medicare regulations.
• Coordinates the Plan of Care with the clinical team to ensure appropriate utilization of services.
• Ensures pain assessments and CAAs care plans are addressed.
• Conducts utilization reviews, including Medicare NOMNC/ABN Letters, Medicare Certifications, and hospital referral reviews.
• Coordinates information with other departments when coding MDS.
• Provides ongoing education for initial RNAC certification and recertification requirements.
• Takes direction from the Regional RNAC and supports recommendations to work as a team.
• Develops and maintains a good working rapport with interdepartmental personnel and other departments within the facility.
• Maintains the confidentiality of all resident care information.
• Participates in weekly CMI meetings.

• Must have a current RN license in the Commonwealth of Pennsylvania and RNAC Certification.
• Must have MDS experience.
