We are seeking a dedicated and empathetic Registered Nurse to become an integral part of our hospice program at Hospice of the Piedmont Inc.. In the role of RN Case Manager, you will provide exceptional care to patients and their families during their final stages of life.
Key Responsibilities- Perform comprehensive nursing assessments and create tailored care plans.
- Work in collaboration with physicians and interdisciplinary team members to ensure patients experience optimal comfort and quality of life.
- Educate patients and their families regarding disease progression, symptom management, and available end-of-life care options.
- Coordinate with healthcare professionals to deliver holistic services.
- Maintain precise documentation and actively participate in team meetings and care planning.
- Stay informed about best practices in hospice care and pursue professional development opportunities.
- Active RN license in West Virginia is required.
- A minimum of 2 years of nursing experience, ideally in hospice or palliative care settings.
- Familiarity with hospice regulations and standards of care.
- Excellent clinical assessment and communication abilities.
- Capability to work both independently and collaboratively within a team.
- Demonstrated compassion and empathy in patient interactions.
- Highly organized, self-driven, and proficient in electronic medical record systems.
