Job Summary

We are seeking a skilled Advanced Practice Provider to join our team at AbsoluteCare. As a Community Based provider, you will be responsible for delivering high-quality medical care to patients in their homes, working collaboratively with our care team to ensure seamless transitions and optimal outcomes.

Key Responsibilities
  • Conduct comprehensive in-home assessments, including medical history reviews, physical examinations, and medication management.
  • Work collaboratively with physicians, case managers, social workers, and other healthcare professionals to develop and implement patient-centered care plans.
  • Provide education and coaching to patients on medication adherence, chronic disease self-management, and other health-related topics.
  • Monitor patients at risk of being lost to care and implement interventions to prevent loss.
Requirements
  • Certified Registered Advanced Practice Provider in Adult Medicine with Acute Care experience.
  • ACLS certification.
  • Ability to take a creative and innovative approach to problem-solving to aid patients in overcoming barriers to care.
  • Excellent computer skills, including knowledge of Microsoft Office.
  • Patient-centered focus.
Preferred Qualifications
  • Experience in a Patient Centered Medical Home.
  • Experience working with high-risk and medically complex patients with multiple comorbidities.
