Job Summary

We are seeking a highly motivated and detail-oriented Population Health Coach to join our team. The successful candidate will provide programmatic and client support for the provision of patient-centered care, working closely with healthcare professionals and community resources to ensure seamless care coordination.

Key Responsibilities
  1. Complete intake assessments to identify unmet social services and healthcare needs, and develop effective referral and linkage strategies to address these needs.
  2. Assist clients with healthcare and medication insurance applications, as well as housing applications, to ensure access to necessary resources.
  3. Develop and maintain knowledge of local and state resources for patient assistance programs, including medical, food, transportation, and housing services.
  4. Encourage continuity of care between clinicians and patients by providing individual support and assistance in navigating the healthcare system.
  5. Collaborate with healthcare professionals to develop and implement effective care plans, and provide education and support to patients and families.
Requirements
  • Minimum of 3 years of experience providing healthcare services and/or community outreach for applicable populations.
  • Knowledge of health promotion and counseling, preferably with a focus on disease and lifestyle-related topics.
  • Strong interpersonal skills and ability to work collaboratively with diverse populations.
  • Associates degree in health science or equivalent healthcare experience; Bachelor's degree preferred.
  • Applicable specialty certification(s) and clinical trainings/courses as applicable.
