Job Title: Licensed SW Specialist

CAMcare Health Corporation is seeking a highly skilled Licensed SW Specialist to join our team. As a key member of our healthcare team, you will provide essential social work services to patients and their families, promoting their overall well-being and quality of life.

Job Summary:

The Licensed SW Specialist will work closely with our healthcare team to develop and implement comprehensive social work plans, addressing the unique needs and challenges of each patient. This role requires a strong understanding of social work principles, excellent communication skills, and a passion for delivering exceptional patient care.

Key Responsibilities:
  • Provide social services and assistance to improve the social and psychological functioning of patients and their families.
  • Assess and reassess patient's support systems, available community resources, and other factors to plan, develop, and implement an appropriate plan of care.
  • Operate as an advocate with all community populations, utilizing skills that respect issues of cultural and ethnic diversity and equity for every patient and family.
  • Coordinate services to Health Center, community agencies, and other professionals, maintaining working knowledge of and liaison with community agencies.
  • Assume responsibility for continued professional development, keeping current with social work and healthcare developments and seeking to increase further enhancement of job-related knowledge.
  • Assist patients, families, or vulnerable populations with the psychosocial support needed to cope with health, social, and psychological concerns.
  • Collaborate with the Health Care Team and involves the patient/family in the development and implementation of plans.
  • Provide psychosocial assessments, intervention, and care plan to patients of the Health Center and assists them and their families in coping with their illness, diagnosis, and/or life situation and meet the continuing healthcare needs.
  • Serve as a patient advocate, maintaining a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child and elder abuse.
  • Utilize the EMR system, maintains records and statistics in accordance with department policies.

  • Attend and participate in staff committees, departmental clinical collaboratives, and other administrative meetings.
  • Contribute to the efficiency and effectiveness of the Health Center's service to patients by offering suggestions and directing or participating as an active member of the Health Care Team.
  • Perform other related duties as assigned.
Requirements:
  • A Master's Degree in Social Work from an accredited university or college.
  • Licensed Social Work (LSW) OR Licensed Clinical Social Work (LCSW) in New Jersey.
  • One to three years' experience as a social worker in a community healthcare environment, medical center, or other related field.
  • Bi-lingual (English/Spanish) is preferred.
