Job Summary

Wellbe Senior Medical is seeking a highly skilled and compassionate Nurse Practitioner to join our team as an Engagement APP- Nurse Practitioner. This role is critical to our overall interdisciplinary team and focuses on providing compassionate care to patients with chronic and acute illnesses and injuries while supporting their ability to stay in their own home.

Key Responsibilities
  • Model the Wellbe values and mission in care delivery for patients
  • Follow rules and regulations of state and federal regulatory agencies and certified agencies
  • Deliver primary care in the home-based setting for patients
  • Complete initial patient screenings/physical examinations, create assessment/plan, and document in a timely manner
  • Order, interpret, and analyze diagnostic tests and determine need for follow-up appointments and further treatment options
  • Prescribe and manage medications and treatments as appropriate
  • Update patient records and check for accuracy at each patient appointment
  • Deliver highest standard of quality care while maintaining company metrics
  • Focus on clinical care delivery to support necessary items for documentation/coding, HEDIS measures, engagement of patients, and managing medical expenses
Requirements
  • Master's Degree in Nurse Practitioner studies from an accredited program
  • Active, unencumbered Registered Nurse and Nurse Practitioner licenses in the posted state
  • Current board certification, either AANC or AANP
  • BLS certified and active NPI# as an NP
  • Active, unencumbered DEA licensure or eligible to apply for upon hire
  • Previous experience in home care, primary care, hospice, palliative care, or geriatrics preferred
Preferred Skills and Abilities
  • Ability to conduct in-home setting visits
  • Demonstrated role proficiency with Microsoft Office and Electronic Health Records
  • Role model in courageous teamwork, integrity, ethics, and productivity
  • Experience in diagnosing and treatment of ongoing, chronic, and urgent medical/psychological conditions of older adults
  • Experience in panel management, primary care, geriatrics, internal medicine, palliative care
Work Conditions

Ability to lift up to 30 lbs. Moving, lifting, or transferring of patients may involve lifting of up to 50 lbs. Ability to stand for extended periods. Ability to drive to patient locations. Fine motor skills/Visual acuity.
