Job Summary

We're seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a Geriatric Care Nurse Practitioner, you'll be responsible for delivering primary, urgent, and acute medical care to our patients in their homes. You'll work closely with our interdisciplinary team to provide concierge-level care and support to our patients and their families.

Key Responsibilities
  • Provide primary care to patients in their homes, including routine check-ups, medication management, and treatment of acute illnesses
  • Collaborate with our team to develop and implement individualized care plans that meet the unique needs of each patient
  • Conduct initial patient screenings and physical examinations, and create appropriate assessment and treatment plans
  • Order and interpret diagnostic tests, and determine the need for follow-up appointments and further treatment options
  • Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents
  • Update patient records and ensure accuracy at each patient appointment
  • Deliver quality care while maintaining all company metrics pertaining to documentation, coding, HEDIS measures, patient engagement, and medical expense
  • Participate in regularly scheduled team meetings and collaborate with the interdisciplinary team to achieve our goals
Requirements
  • Master's degree in Nursing
  • Valid nurse practitioner or physician assistant license in the state of operations and current board certification in your field
  • 5+ years of progressive care experience
  • BLS and ACLS certification and an NPI# as an NP
  • Valid driver's license, car insurance, and access to a car for patient visits
