Job Summary

We are seeking an experienced Nurse Practitioner to join our team of healthcare professionals. As a key member of our geriatric care team, you will be responsible for delivering primary, urgent, and acute medical care to seniors in their homes.

Responsibilities
  • Provide comprehensive medical care to patients in their homes, including routine check-ups, urgent care, and acute medical interventions.
  • Collaborate with our interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.
  • Conduct initial patient screenings, physical examinations, and create assessment plans to address urgent care needs.
  • Order, interpret, and analyze diagnostic tests, and determine the need for follow-up appointments and further treatment options.
  • Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents.
  • Update patient records and ensure accuracy at each patient appointment.
  • Deliver quality care while maintaining all company metrics pertaining to documentation, coding, HEDIS measures, patient engagement, and medical expense management.
Requirements
  • Master's degree in Nursing and valid nurse practitioner or physician assistant license in the state of operations.
  • Current board certification in your field and 5+ years of progressive care experience.
  • BLS and ACLS certification and an NPI number as an NP.
  • Valid driver's license, car insurance, and access to a car for patient visits.

About the Position

This is a full-time, permanent position with a regular shift, 8 am to 5 pm, Monday to Friday. The role involves traveling to patient homes and locations, with occasional on-call rotation.
