Job Summary

PACE Southeast Michigan seeks a skilled Master's Social Worker to join our team as a Care Coordinator. In this role, you will work collaboratively with our interdisciplinary team to assess the needs of our participants and deliver high-quality social services.

Key Responsibilities
  • Conduct comprehensive bio-psychosocial assessments to identify participant needs and develop individualized care plans.
  • Provide counseling, education, and consultation services to participants and caregivers, utilizing evidence-based techniques such as motivational interviewing and cognitive behavioral therapy.
  • Direct case management, addressing issues related to housing, finances, safety, and psychiatric concerns.
  • Collaborate with the interdisciplinary team to develop and communicate plans of care, changes in care, and progress.
  • Evaluate participant outcomes and communicate this information among team members.
Qualifications
  • Master's degree in Social Work from an accredited program.
  • Licensed to practice in the State of Michigan (Limited License may be considered depending on experience).
  • One year of experience working as a clinical social worker, preferably in geriatrics.
  • Demonstrated knowledge and skill in evaluating, planning, implementing, and monitoring patient-centered approaches to care.
  • Cultural competence and ability to care for a diverse population.
  • Proficient in electronic health record systems and Microsoft Office applications.
