Key Responsibilities

As a part of our team, you will:

  • Evaluate individuals residing in nursing facilities to determine their potential for community living with appropriate support services.
  • Create personalized care plans that focus on the individual’s needs and monitor their progress in accordance with program standards.
  • Facilitate the discharge process and coordinate the transition to community living alongside clients, nursing facility personnel, health plans, and other support systems.
  • Provide ongoing care management after clients transition to their new living arrangements, including coordinating benefits, in-home services, and community resources, as well as scheduling medical appointments and advocating for clients.
  • Collaborate with partner organizations to ensure a seamless transition experience.
  • Adhere to all program protocols and guidelines.

Qualifications

  • Master's Degree (MA, MSW) in social work, counseling, psychology, or a related field
  • Registered Nurse (RN) certification
  • Preferred: Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC)
  • Minimum of one year of experience in social services
  • Experience working with clients facing severe mental health challenges, substance abuse issues, or complex physical health conditions is preferred
  • Strong written and verbal communication skills, with the ability to engage effectively with clients
  • Proficient in Microsoft Office Suite and capable of learning proprietary software
  • Knowledge of community support services
  • Cultural sensitivity and the ability to work with diverse populations
  • Current driver's license and access to a reliable vehicle
  • Preferred: Experience with Community Care Program (CCP) or similar care management programs
  • Preferred: Familiarity with Colbert and/or Williams consent decrees
