Key Responsibilities
As a part of our team, you will:
- Evaluate individuals residing in nursing facilities to determine their potential for community living with appropriate support services.
- Create personalized care plans that focus on the individual’s needs and monitor their progress in accordance with program standards.
- Facilitate the discharge process and coordinate the transition to community living alongside clients, nursing facility personnel, health plans, and other support systems.
- Provide ongoing care management after clients transition to their new living arrangements, including coordinating benefits, in-home services, and community resources, as well as scheduling medical appointments and advocating for clients.
- Collaborate with partner organizations to ensure a seamless transition experience.
- Adhere to all program protocols and guidelines.

Qualifications
- Master's Degree (MA, MSW) in social work, counseling, psychology, or a related field
- Registered Nurse (RN) certification
- Preferred: Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC)
- Minimum of one year of experience in social services
- Experience working with clients facing severe mental health challenges, substance abuse issues, or complex physical health conditions is preferred
- Strong written and verbal communication skills, with the ability to engage effectively with clients
- Proficient in Microsoft Office Suite and capable of learning proprietary software
- Knowledge of community support services
- Cultural sensitivity and the ability to work with diverse populations
- Current driver's license and access to a reliable vehicle
- Preferred: Experience with Community Care Program (CCP) or similar care management programs
- Preferred: Familiarity with Colbert and/or Williams consent decrees
