Clinical Pharmacy Specialist

We are seeking a highly skilled Clinical Pharmacy Specialist to join our team at MD Anderson Cancer Center. As a Clinical Pharmacy Specialist, you will play a critical role in optimizing the pharmacotherapeutic aspects of patient care, collaborating with the multidisciplinary healthcare team to develop and implement effective medication therapy plans.

Key Responsibilities:

• Develop and maintain an active clinical pharmacy practice on assigned services, participating in inpatient rounds and outpatient clinic coverage, including on-call and weekend coverage as assigned.
• Collaborate with the healthcare team to optimize pharmacotherapeutic aspects of patient care, managing medication therapy for individual patients and developing, recommending, implementing, monitoring, and redesigning drug therapy regimens to optimize therapy.
• Manage daily patient medication problems and emergency situations, documenting activities in the electronic health record appropriately.
• Provide timely follow-up on the clinical effects of medication regimens, making adjustments as necessary to achieve optimal outcomes and documenting actions.
• Provide drug and clinical information to other healthcare professionals, developing and communicating specific pharmacotherapeutic plans for individual patients.
• Participate in discharge planning for assigned inpatients, proactively assessing patient financial status and addressing barriers.
• Adhere to regulatory/compliance policies and procedures governing the profession and scope of practice, completing pharmacy protocol reviews to facilitate IRB submission processes.
• Critically analyze practice, experience, medical literature, and other information resources for self-improvement, obtaining/maintaining required and preferred licenses, certifications, training, and continuing education prior to deadlines.
• Consistently document and review clinical activity/interventions appropriately, using professional judgment and language to report safety events utilizing institutional and federal government systems.
• Model service excellence, attending and actively participating in committees/meetings, demonstrating effective oral and written communication skills, and aligning patient care activities and project work with departmental and institutional policies, business model/needs, goals, and vision.

Requirements:

• Pharm.D degree
• Texas State Board of Pharmacy Pharmacist license within 90 days of job entry
• Board Certification in appropriate specialty area
• Pharmacy Practice Residency (PGY 1) and Residency in the area of specialization (PGY 2)
• Experience as a clinical pharmacist in the area of specialization may substitute for the required residency on a one-to-one basis
