Hours: 32 hours per week

Job Summary:

The Registered Nurse II is a skilled and compassionate healthcare professional responsible for delivering high-quality patient care to individuals and families in the hospice setting. As a key member of the interdisciplinary team, the RN II will work collaboratively with other healthcare professionals to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.

Responsibilities:

  • Conduct comprehensive assessments of patients and families to identify their unique needs and develop individualized care plans.
  • Provide direct patient care, including administering medications, performing treatments, and providing emotional support and education to patients and families.
  • Collaborate with other healthcare professionals to coordinate care and ensure seamless transitions between settings.
  • Document patient information accurately and thoroughly, using electronic medical records and other documentation tools.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote high-quality patient care.
  • Provide education and support to patients and families, including information about disease management, treatment options, and end-of-life care.
  • Develop and maintain relationships with patients, families, and other healthcare professionals to ensure continuity of care and promote a culture of compassion and respect.

Requirements:

  • Current licensure as a Registered Nurse in the State of Maine.
  • 1 year of experience in hospice or palliative care.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to providing high-quality patient care.
  • Basic Life Support (BLS) certification.
