Position: Case Manager
The Case Manager plays a crucial role in providing care coordination services to individuals with a history of substance abuse, functioning as an integral member of the interdisciplinary treatment team.
Key Responsibilities:
- Deliver case management and crisis intervention services to facilitate treatment coordination aligned with workforce initiatives, enhancing access to transitional support for clients facing barriers to employment and self-sufficiency.
- Lead support groups focused on life skills development.
- Advocate for clients to secure necessary services.
- Engage with relevant agencies and systems (such as DSS, probation, and courts) to assist clients in fulfilling all mandates.
- Provide ongoing case management addressing housing, transportation, childcare, food, and other essential needs.
- Conduct toxicology screenings as required.
- Maintain communication with treatment planning and direct service providers, as well as Suffolk County vocational service providers.
- Submit weekly schedules, obtain prior approval for off-site activities, and monitor effective time utilization.
- Participate in staff and community meetings as appropriate.
- Offer crisis intervention and group work services, providing backup coverage when necessary.
- Utilize computer skills essential for optimal job performance.
- Ensure quality care delivery in accordance with the agency's Medicaid Compliance Plan and accountability systems.
- Report any non-compliance issues to the compliance officer or supervisory structure.
- Each role at Outreach has specific production and outcome goals, which will be discussed during annual evaluations.
Supervision:
This position is supervised by the Assistant Program Director in collaboration with the Program Director.
Qualifications:
- Preferred qualifications include CASAC-T, CASAC, or higher.
- Minimum of one year of experience working with clients with substance abuse disorders.
- Experience with adolescents facing co-occurring mental health issues is preferred.
- Experience in an OASAS-funded facility is a plus.
- Commitment to delivering quality services and achieving outcome-driven performance metrics.
- Strong organizational management skills with the ability to mentor staff and develop high-performance clinicians.
- Excellent networking and public relations skills to engage diverse stakeholders.
- Action-oriented, adaptable, and innovative approach to program planning.
Collaboration:
Ability to work effectively with diverse groups of individuals.
Position Status:
This is a part-time, non-exempt role.
Work Environment:
This position operates in a professional office setting and in the field. Standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines will be utilized. Travel among assigned sites may be required.
