VNS Health is seeking a skilled and experienced Registered Nurse (RN) to fill a Per Diem Field Supervisor position in our Home Health department. As a Field Supervisor, you will play a critical role in delivering high-quality patient care and ensuring the success of our home health programs.

Key Responsibilities
  • Case Management Support: Provide clinical intervention, develop care plans, and follow-up support to patients in their homes.
  • Leadership and Mentorship: Serve as a mentor and leader to Home Health Aides, ensuring they receive the training and support they need to provide exceptional patient care.
  • Documentation and Communication: Extensively document patient visits in the Electronic Medical Record (EMR) and convey significant changes in patient condition to the clinical care team.
  • Supervision and Orientation: Orient and supervise Home Health Aides in the field, ensuring they are equipped to provide high-quality patient care.
Requirements
  • Current RN License: Must be licensed to practice as a Registered Nurse in New York State.
  • Home Care Experience: Minimum of two years of experience as an RN in a home care setting.
  • Valid Driver's License: May be required.
