Job Overview

We are seeking a Registered Nurse to join our Home Health team as a Nurse Care Manager. This role is an excellent opportunity for a skilled nurse to provide comprehensive care management services to patients and families, promoting optimal health outcomes and quality of life.

Key Responsibilities

  • Evaluate and develop individualized care plans based on established standards and patient needs.
  • Provide patient and family education to support recovery and engagement in the care process.
  • Regularly assess and document patient progress and responses to interventions.
  • Coordinate care activities with other members of the healthcare team to ensure seamless delivery of services.
  • Ensure accurate and efficient execution of physicians' orders and timely communication of adverse or sudden changes in patient condition.

Requirements

  • Graduate from an accredited nursing program.
  • Current RN Washington State license.
  • Current Basic Life Support certification.
  • Oral and written fluency in the English language.

Salary and Benefits

This position offers a competitive salary range of $85,000 - $110,000 per year, depending on experience, plus a comprehensive benefits package, including medical, dental, and retirement benefits, and paid time off.
