Job Summary

We are seeking a skilled Case Manager to join our Health Home Program team. The successful candidate will work closely with clients to break through barriers and achieve their goals, while also advocating for their needs and rights. This is a full-time position, requiring 35 hours of work per week.

Key Responsibilities
  • Conduct initial intake and periodic reassessments of clients, identifying their strengths and challenges.
  • Develop and implement individualized service plans, working with clients to set and achieve goals.
  • Monitor clients' progress, providing regular support and guidance to help them overcome obstacles.
  • Act as a liaison and advocate for clients, connecting them with community resources and services.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support for clients.
Requirements
  • Associate's degree or equivalent experience required.
  • Bi-lingual English with Spanish, Russian, Haitian-Creole, or Chinese (Mandarin or Cantonese) preferred.
  • Knowledge of HIV/AIDS and related issues, as well as experience working with diverse populations.
