Job Summary

Rogue Community Health is seeking a skilled RN Nurse Case Manager to join our team. As a key member of our clinical staff, you will play a vital role in ensuring the efficient and effective flow of patients during scheduled clinics.

Key Responsibilities
  • Utilize the nursing process to provide patient care and education, focusing on patient engagement and behavioral change, treatment compliance, and health risk factor reduction.
  • Work collaboratively with clinic staff to improve the management and stability of medically complex patients, resulting in reduced ER visits, hospitalizations, treatment-related complications, and improved patient quality of life.
  • Develop and implement individualized care plans to address the unique needs of each patient.
  • Provide education and support to patients and their families to promote healthy behaviors and improve health outcomes.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between healthcare settings.
