LPN Crisis Care Role Overview

The LPN Crisis Care position at Ohio's Hospice is a critical role that requires compassion, empathy, and exceptional nursing skills. As a Crisis Care LPN, you will provide high-quality care to patients and their families in their homes or facilities, working under the direction and supervision of an RN.

Key Responsibilities:
  • Provide superior clinical care to patients and their families
  • Work collaboratively with the healthcare team to develop and implement individualized care plans
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement effective care plans to meet patients' needs
  • Document patient care and progress accurately and thoroughly
Qualifications:
  • LPN licensure in the State of Ohio
  • One year of nursing experience preferred
  • CPR certification
  • Computer skills sufficient to properly document services and care
  • Ability to drive during daytime, nighttime, or inclement weather
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage
