Overview

Catholic Health Initiatives is seeking a highly skilled Advanced Practice Provider to join our Nephrology team. This is an excellent opportunity for a motivated and compassionate healthcare professional to deliver high-quality patient care in a dynamic and supportive environment.

Responsibilities
  • Patient Care:
    • Diagnose and treat various medical conditions, including those related to kidney disease and hypertension.
    • Develop and implement comprehensive treatment plans tailored to individual patient needs.
    • Conduct thorough medical examinations and histories to ensure accurate diagnoses and effective care.
  • Communication:
    • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
    • Provide clear and concise medical information to patients and their families.
    • Foster a collaborative and patient-centered approach to healthcare, prioritizing empathy and understanding.
  • Documentation:
    • Maintain accurate and up-to-date patient records, adhering to strict confidentiality and security protocols.
    • Document medical findings, diagnoses, and treatment plans in a clear and concise manner.
    • Ensure compliance with coding and billing regulations to optimize reimbursement and minimize errors.
  • Diagnostic Procedures:
    • Order and interpret diagnostic tests, such as lab work and imaging, to inform treatment decisions.
    • Perform procedures as needed, collaborating with specialists when necessary.
    • Collaborate with specialists for further diagnostic evaluation and treatment planning.
  • Team Collaboration:
    • Collaborate with nurses, specialists, and other healthcare professionals to ensure comprehensive and coordinated care.
    • Participate in interdisciplinary team meetings and case discussions to share knowledge and best practices.
    • Provide consultative services to other healthcare providers, promoting a culture of collaboration and mutual respect.
  • Quality Improvement:
    • Participate in quality assurance and improvement initiatives to enhance patient care and safety.
    • Review and analyze clinical outcomes for continuous improvement, identifying areas for growth and development.
    • Implement changes to enhance patient care and safety, prioritizing evidence-based practices and patient-centered care.
  • Qualifications:
    • American Board Certified or Board Eligible in Nephrology
    • A valid and unrestricted state medical license or license-eligible
