Position Overview

Molina Healthcare is seeking a dedicated RN Care Transitions Case Manager. The ideal candidate will be responsible for managing care transitions for our Medicaid population, ensuring a seamless process from hospital to home or other care settings.

Key Responsibilities

  • Monitor and support members throughout a 30-day program that initiates at hospital admission and continues through various transitions, aiming to minimize readmissions.
  • Collaborate with hospital discharge planners, healthcare providers, and family networks to facilitate safe and effective transitions.
  • Assess members' needs and ensure appropriate caregiving and medical oversight during transitions.
  • Conduct face-to-face visits with members in hospitals and home settings, particularly focusing on high-risk individuals post-discharge.
  • Utilize the Coleman Care Transitions Model to coordinate care and reassess member needs.
  • Educate members on critical areas such as medication management, follow-up care, and recognizing signs of health deterioration.
  • Employ motivational interviewing techniques to encourage positive health behaviors among members.
  • Identify barriers to care and provide necessary support to address these challenges.
  • Facilitate interdisciplinary care team meetings to enhance collaboration and care delivery.
  • Provide guidance and education to non-RN case managers as needed.
  • Perform medication reconciliation when necessary.
  • Travel locally to conduct member visits as required.

Qualifications

Education: Graduate from an accredited nursing program; a Bachelor's Degree in Nursing is preferred.

Experience: 1-3 years in hospital discharge planning or home health settings.

Licensure: Must possess an active, unrestricted State Registered Nursing (RN) license and a valid driver's license with a clean driving record.

Preferred Qualifications: Bachelor's Degree in Nursing and 3-5 years of experience in hospital discharge planning or home health. Additional certifications such as Transitions of Care Sub-Specialty Certification or Certified Case Manager (CCM) are advantageous.

Compensation: Molina Healthcare offers a competitive salary and benefits package. Pay range is $51.49 per hour, with actual compensation varying based on experience and location.
