Clinical Care Specialist

Key Responsibilities:
- Assess physical, psychosocial, health education, and discharge needs of patients
- Implement nursing interventions and develop plans of care
- Collaborate with patients and care teams for wrap-around services
- Perform independent visits under standing order procedures
- Educate patients, families, and communities on various health care subjects
- Monitor patients' ongoing conditions and adjust treatment plans as needed
- Provide motivational interviewing and harm reduction counseling
Requirements:
- Associate's degree in Nursing from an accredited college or equivalent combination of education and experience; BSN preferred
- Unencumbered Washington State Registered Nurse license required
- Current BLS certification
