Key Responsibilities
  • Case Management and Evaluation
    • Establish and maintain relationships with families, providing a strengths-based approach to case management and evaluation.
    • Conduct thorough assessments to identify needs and develop personalized service plans.
    • Collaborate with community professionals to implement service plans and ensure families receive necessary support.
  • Communication and Collaboration
    • Develop and maintain positive relationships with community partners, including child welfare professionals, healthcare providers, and social service agencies.
    • Effectively communicate with families, community partners, and other stakeholders to ensure seamless service delivery.
  • Community Resource Development
    • Identify and develop relationships with community resources, including housing and treatment options.
    • Assist families in accessing necessary resources and services.
Requirements
  • Education and Experience
    • High school diploma or equivalent required.
    • 4 years of experience working with at-risk children and families.
    • Demonstrated ability to engage high-risk mothers and their families.
    • Ability to conduct sensitive and confidential interviews.
  • Preferred Qualifications
    • Addiction and Prevention Services (AAPS) Certified.
    • 1 year of experience working with parents with drug and alcohol issues.
    • Bachelor's degree in Social Work or a related field.
    • Individuals with life experience dealing with substance abuse issues or other relevant issues.
