About This Opportunity

HopeHealth is seeking a skilled Registered Nurse to provide hospice services to patients in their homes and long-term care facilities.

Key Responsibilities
  • Represent HopeHealth in assigned settings and serve as a subject matter expert in the hospice benefit.
  • Present hospice to eligible patients and caregivers, highlighting the benefits of the service in caring for individuals with a terminal diagnosis.
  • Perform initial assessments of patients and families to determine the impact of the terminal diagnosis on their physical, functional, psychosocial, and environmental needs and activities of daily living.
  • Complete hospice admissions in compliance with Medicare/Medicaid and state regulations.
  • Develop individualized plans of care, including documentation of the Local Coverage Determination.
  • Participate in weekly hospice interdisciplinary group (IDG) meetings to review patients on service as needed.
  • Document in the electronic medical record (EMR) at the point of care and meet department expectations for timely completion of documentation.
  • Ensure that patient information regarding payer source is current and correct.
  • Complete other duties as assigned, including field evaluations/admissions, triage visits, and GIP coverage at adjacent sites.
Requirements
  • Strong desire to provide patient-centered care in a hospice setting.
  • RN required; BSN preferred.
  • Licensed as a Registered Nurse in Rhode Island and Massachusetts.
  • Minimum of 1-3 years of clinical experience.
  • Excellent nursing assessment, clinical, and communication skills.
