Palliative Care Nurse

Job Summary
The palliative care nurse provides comprehensive care to patients with serious illnesses, focusing on relief from pain and symptoms. This role requires a registered nurse with experience in community health and a strong understanding of patient education and support.
Key Responsibilities
1. Conduct thorough assessments of patients and families to determine care needs.
2. Develop and implement individualized care plans that address physical, emotional, and spiritual needs.
3. Collaborate with interdisciplinary teams to ensure seamless care coordination.
4. Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
5. Maintain accurate and detailed records of patient interactions and care provided.
Requirements
1. Graduate of a National League for Nursing accredited school of nursing.
2. Minimum of two years of experience in public health, home care, or hospice nursing.
3. Current licensure to practice professional nursing in the state.
4. Excellent observation, problem-solving, and communication skills.
5. Ability to work independently and prioritize tasks effectively.
Compensation
$55-65/hr.
