Care Coordinator Specialist

In this role, you will be responsible for providing comprehensive case management services, including crisis intervention, advocacy, and assistance with problem-solving. Your strong interpersonal skills and ability to work as part of a team will enable you to build productive relationships with our clients and external partners.
As a Care Coordinator Specialist, you will assess client needs and develop personalized care plans to achieve self-sufficiency. You will conduct field work, including home visits and escorting clients to appointments, and execute care activities in accordance with client goals and health home acuity rating.
We offer a competitive salary of $65,000 - $85,000 per year, depending on experience. Our benefits package includes comprehensive health insurance, retirement savings plan, and paid time off.
