Job Description">">As a Patient Care Specialist with the Office for People with Developmental Disabilities, you will play a critical role in supporting individuals with intellectual and developmental disabilities in achieving their full potential. Your responsibilities will include providing direct patient care, developing and implementing individualized care plans, and participating in interdisciplinary team meetings to discuss client progress and develop new care strategies. You will also maintain accurate and up-to-date records of client interactions, treatments, and outcomes, ensuring seamless coordination of care across various settings and teams.

About the Position">">Key Responsibilities:

* Provide direct patient care, including administering medications, performing vital signs, and implementing treatment plans
* Develop and maintain relationships with clients, families, and healthcare professionals to ensure seamless coordination of care
* Participate in interdisciplinary team meetings to discuss client progress and develop new care strategies
* Maintain accurate and up-to-date records of client interactions, treatments, and outcomes

">Qualifications:">

* Current licensure as a Licensed Practical Nurse (LPN) in New York State
* 1+ years of experience in patient care, preferably in a long-term care or rehabilitation setting
* Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and teams
* Ability to prioritize tasks, manage time, and meet deadlines in a fast-paced environment
