Job SummaryWe are seeking a highly skilled and compassionate Medical Social Worker to join our team at Amedisys. As a Medical Social Worker, you will play a vital role in providing social work services to patients and families, assisting with health-related financial, social, and emotional concerns, and promoting effective coping skills.
Responsibilities- Assist patients and families with health-related financial, social, and emotional concerns and assist with effective coping skills.
- Provide social work services in accordance with the plan of care.
- Ensure that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
- Educate patients and family members and other team members on medical social work interventions and treatment plans.
- Perform care planning, transfer, and discharge functions.
- Identify risks or threats to the patient's health in the patient's environment and promptly implement appropriate interventions to reduce risks.
- Communicate identification of risks/threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.
- Holds a master's or doctorate degree from a school of social work, accredited by the Council on Social Work Education.
- Has one year of social work experience in a healthcare setting.
- Meets state and federal requirements for this position.
- Current CPR certification.
