Job Description

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our Primary Care Clinic team. As a key member of our clinical team, you will have the opportunity to make a meaningful difference in the lives of our patients and contribute to the delivery of exceptional patient care.

Key Responsibilities
  • Partner with our clinical team to provide high-quality patient care, ensuring seamless communication and coordination across all aspects of care.
  • Perform a wide range of patient care services under the direct supervision of a registered nurse, advanced practice nurse (APN) or physician, including therapeutic and diagnostic procedures.
  • Implement and document patient and family education as delegated by the RN or physician, promoting informed decision-making and empowerment.
  • Obtain and record vital signs, perform tests, and document all aspects of patient care according to hospital policies and procedures.
  • Assist with collection of specimens, perform phlebotomy services, and label and log specimens in compliance with hospital standards.
Required Qualifications
  • A completed High School diploma or equivalent from an accredited body.
  • A current certificate of cardiopulmonary resuscitation training.
  • Must have phlebotomy experience.
  • Experience working in a community health setting.
  • Completion of a certified/registered program for medical assistants that is recognized by the American Heart Association or American Red Cross.
Preferred Qualifications
  • Considerable skill in communicating verbally, in determining priorities, and in organizing work, with the ability to maintain an organized, alphabetized filing system.
  • General working knowledge of PC, Excel, Microsoft Word for Windows, OASIS, and EPIC.
  • Computerized appointment scheduling experience.
  • Ability to handle stress in a fast-paced environment.
  • Considerable skill in handling sensitive public contacts with discretion, tact, and courtesy when dealing with patients, coworkers, and physicians.
  • Physical ability to perform stocking, lifting, and transporting supplies and instruments.
Salary and Benefits

We offer a competitive salary range of $45,000-$60,000 per year, depending on experience, as well as a comprehensive benefits package, including medical, dental, and vision coverage, retirement plan, and paid time off.

Work-Life Balance

As a Clinical Care Coordinator, you will have the opportunity to work a regular Monday-Friday schedule, with approximately 40 hours per week, and no on-call shifts. You will be scheduled to work 100% at our Hyde Park Clinic, with the opportunity to contribute to a dynamic and supportive team environment.
