Job Summary

We are seeking a highly skilled Wound Care Nurse Practitioner to join our team at Hackensack Meridian Health. The successful candidate will be responsible for providing high-quality patient care, utilizing the patient-centered coordinated care model.

Key Responsibilities
  • Provide medical assessments, treatments, and clearances for patients, employees, and external customers, adhering to company profiles and regulatory standards.
  • Perform clinical care professionally, serving as a customer advocate and role model, and ensuring that practice accurately reflects NJ Board of Medical Examiners regulations, Code of Ethics, and current NJ Department of Health requirements.
  • Develop and implement appropriate plans of care, facilitate patient teaching and support programs, and evaluate current protocols and standards of care for translation into clinical practice.
  • Apply evidence and value-based practices to achieve positive outcomes for patients, provide medication safely and effectively, and prescribe medications as appropriate.
  • Collaborate with physicians, consult effectively with other healthcare team members, and demonstrate effective communication skills to achieve results.
  • Provide accurate, systematic, and organized documentation of findings, recommendations, and education, and participate in continuing education and maintain documentation.
  • Develop a personal plan of development in area of clinical expertise, confer with leader on goals and objectives, and provide culturally diverse and educationally appropriate education in relation to patient care and needs.
  • Serve as a clinical resource to nursing staff, patients, and other multidisciplinary team members across the continuum, evaluate complex patients, and identify appropriate interventions and evaluation of outcomes.
  • Participate in policy/procedure reviews for updating with current evidence-based and value-based practices, and act as a resource to staff for addressing clinical, process, and system opportunities for improvement.
  • Promote a collaborative work environment that supports the organizational mission, behavior expectations, and values, and promote innovation within clinical practice by seeking opportunities to bring new clinical knowledge into practice.
  • Identify ethical care issues and coordinate interdisciplinary collaborative meetings to resolve those issues, and float to various clinic locations as needed.
Qualifications
  • Graduate of an NLN/AACN accredited program in nursing.
  • Master's Degree.
  • Maintain clinical competencies through participating in continuing educational programs and clinical practice as mandated by Board of Nursing and national certification body.
  • Experience in relevant patient care setting or as regulations dictate.
Preferred Qualifications
  • Master's Prepared Advanced Practice Nurse.
  • Minimum of 3 years professional experience.
  • Strong organizational and computer skills.
  • Effective verbal and written communication skills.
  • Emotional intelligence, critical thinking, and effective interpersonal skills.
  • Ability to adapt to multiple and changing priorities.
Licenses and Certifications
  • NJ State Professional Registered Nurse License.
  • Advanced Practice Nurse Certification (with Master's Degree).
  • AHA Basic Health Care Life Support HCP Certification.
