About the Role

Pinnacle Home Care is seeking a skilled Registered Nurse to join our team as a Home Health Nurse. As a key member of our team, you will play a vital role in delivering high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Conduct thorough assessments of patients to determine their care needs and develop personalized plans to meet those needs.
  • Provide direct patient care, including administering medications and treatments as prescribed by physicians.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Supervise and evaluate the care provided by home health aides to ensure the highest level of quality and safety.
  • Participate in on-call duties and ensure that arrangements for equipment and other necessary items and services are available.
Requirements
  • Graduate of an accredited school of nursing with a current license in the state and CPR certification.
  • One to two years of recent acute care experience in an institutional setting, with home health care experience preferred.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
