Key Responsibilities- Conduct thorough assessments of patients to determine their care needs and develop personalized plans to meet those needs.
- Provide direct patient care, including administering medications and treatments as prescribed by physicians.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Supervise and evaluate the care provided by home health aides to ensure the highest level of quality and safety.
- Participate in on-call duties and ensure that arrangements for equipment and other necessary items and services are available.
- Graduate of an accredited school of nursing with a current license in the state and CPR certification.
- One to two years of recent acute care experience in an institutional setting, with home health care experience preferred.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
